Life’s Journey Hospice offers inpatient care at our facility, home hospice care, or palliative care.

At Life's Journey Hospice, we understand the profound impact of loss and the importance of comprehensive care beyond physical well-being. Our dedicated team is here to provide support every step of the way, offering bereavement services to help individuals and families navigate the grieving process.

Our skilled social workers are trained to offer emotional support, guidance, and resources to assist in coping with the challenges that follow loss. Additionally, our chaplains provide spiritual care and comfort, offering solace and a listening ear to those seeking peace and understanding during moments of reflection.

As the director of bereavement services, I work with those facing loss. Together we navigate the pathway of grief.  Because no two experiences are the same, we all grieve in our unique way.  Overcoming the overwhelming feeling of hopelessness can be an arduous task and require a great deal of effort and time. Our support group breaks down grief and many of its components and we work toward a sense of normality, new as it may be. I work with individuals, families or group settings to provide help and hope.

In-Patient Hospice House

We designed our facility to provide a home-like serene comfortable environment with 24-hour nursing care. In our facility, families can spend valuable time together and allow us to be caregivers.

  • Family members and other loved ones can spend the night in our large (over 400 sq ft) private rooms

  • Each room accesses its own private patio, on which the bed if needed, can be wheeled out so the patient can enjoy the sun on their face or the wind in their hair if desired

  • Each room is set up like a studio apartment with its own private bath, kitchen area, and table and chairs

  • The beds can be lowered into a “chair” position so if someone can’t get out of bed they can still feel as though they are part of the group

Home-Hospice

Our hospice is more than the hospice house. We can bring that same great care to wherever you call home! Life’s Journey meets you where you are to manage the symptoms, pain, and spiritual and emotional needs of the patient and their family.

Hospice is Medicare Part A covered and listed below is the range of services that Medicare covers:

  • Nursing Caregiver

  • Medical Social Worker

  • Physician and NP

  • Home health aide

  • Counseling (including dietary, pastoral, and others)

  • Medical equipment and supplies

  • Bereavement services for families

Palliative Care

Palliative care is specialized medical care for people living with a serious illness. This type of care is focused on providing relief from the symptoms and stress of the illness. The goal is to improve the patient's and family's quality of life. Palliative care is provided by a specially-trained team of doctors, nurses, and other specialists who work with a patient’s other doctors to provide an extra support layer. Palliative care is based on the needs of the patient, not on the patient’s prognosis. It is appropriate at any age and stage in a serious illness, and it can be provided with curative treatment.
